Meeting called to order at 7:05 pm.
7:00 PM: Town of Cohasset Manhole Rehabilitation: NOI #15-26
In attendance to represent agenda item: Dan Coughlin, CES, Board of Sewer Commission
This project involves upgrades and repairs to existing manhole covers throughout the town. The project
will be funded through sewer rates and a bond and will go out to bid_ The new covers are more resilient
and hinged for easier access.
MOTION: By Member Creighton to close the public hearing for Notice of Intent#15-26 and issue
an Order of Conditions for Townwide Manhole Rehabilitation project.
SECOND: Member Graham

7:15 PM: 79 Beechwood Street Sullivan: SF home upgrade: NOI#15-27& SWP#15-39.
In attendance to represent agenda item: Bill and Christine Sullivan, Darren GradylGrady Consulting
This project involves a total rehabilitation of an existing home including a septic upgrade to replace an
existing cesspool. PS feels there are no variances required and that the project as presented will be an
improvement to the existing wetlands area.
MOTION: By Member Pimpare to close the public hearing for Notice of Intent#15-27 and issue an
Order of Conditions for the project at 79 Beechwood Street.

Documents Presented at meetin : Septic Repair--53 Old Coach Road, Revised
The homeowner needs an upgrade to meet Title V requirements. The upgrade includes a new tank and
pump chamber and raising the existing leaching field above groundwater level.
MOTION: By Member Koines to close the public hearing for Request for Determination#15-16 and
issue a Negative 3 determination for the project at 53 Old Coach Road.

In attendance to represent agenda item: Paul and Elizabeth Migliaccio, Carmen Hudson/Cavanaro
The project involves an addition, garage and new driveway. All work will be done outside the 50' buffer
zone and flood plain. There will be no change to the existing home. PS feels the Stornwater application
meets performance standards. Carmen Hudson reported there will be no impact to the buffer zone with
the new driveway and that runoff will flow into a filtration system.
MOTION: By Member Pimpare to close the public hearing for Notice of Intent#15-28 and issue an
Order of Conditions for the project at 7 Buttonwood Lane.

8:00 PM: 35 Jerusalem Road Drive: Beach_St._Builders, Amended OOC—NOI#14-30
In attendance to represent agenda item: Greg Morse/Morse Engineering
The owners want to construct a stone wall along the property line; remove debris and dead growth and
replace with new plantings; extend the wall to the base of ledge along the Isolated Vegetated Wetland
(IVW) and remove vegetation on top of ledge. PS said the IVW is in bad shape and the proposed
enhancement will make the area more functional. Greg Morse said only debris and dead growth will be
removed and that shrubs will be replaced. PV questioned the justification of part of the wall being in the
wetlands but PK felt that elimination of a portion of the wall with replication is ideal. The Commission also
has concerns about work within the IVW. There will be a site visit scheduled for further review of the site.

The applicant wants to put add a 2"d floor onto a non-conforming dwelling located near a Bordering
Vegetated Wetlands with an intermittent stream and Land Subject to Coastal Flooding. PS asked for an
Order of Conditions with a variance to be issued. PK stated the variance is needed because the addition
butts up slightly against the buffer zone. Greg Morse stated the work is 105' from the BVW and PS stated
that all performance standards will be met.

This is new construction. The proposed septic system has stormwater controls, roof drywell systems,
swales along the lot line, a pervious asphalt drive and a maintenance plan all in place. PS sees no
problems with the system as proposed.
MOTION: By Member Vogelman to close the public hearing and issue a Stormwater Permit for the
project at 61 Nichols Road.

8:45 PM: 559 Jerusalem Road Discussion —Chris Ford
In attendance to represent agenda item: Chris Ford/owner; Jim Sandell/architect
This discussion involved positioning of new construction on a developable lot. Two previous hearings
were held on this property for two separate submissions; neither were approved. JC stated that while the
applicant has the right to reopen either hearing, the commission has no application to go by. The owner is
suggesting to build a wider house on a lower ridgeline to minimize impact to abutters. In addition there is
rubble, concrete and other construction debris that the neighbors want moved. Chris Ford said since he is
restricted from using heavy equipment on the property due to previous conditions, he cannot remove the
debris. Both PS and PK said to allow the rubbish removal and amend the condition.
