Meeting called to order at 7:30 pm.
7:30 PM: 70 Whitehead Road—Discussion—can't
In attendance to represent agenda item: Paul Mirabito, Ross Engineering
Green Environmental performed a sail analysis on this property and found evidence of urban fill (metal,
bricks, asphalt) and elevated levels of petroleum and hydrocarbons. A letter dated 10/19/15 from Zoning
Officer Bob Egan states that the fill is in the 100-year Flood Zone and must be removed. Paul Shea is
concerned that the coastal bank is covered by the fill. There is a current Enforcement action on this
property and also fines that have been previously assessed but, to date, no fines have been paid. Paul
Mirabito was asked to come back on December 3 with a definitive strategy for dealing with the violations
at the property.
MOTION: By Member Vogelman to ratify prior Enforcement orders on 70 Whitehead Road.

fines of$300 a day for violations within the wetlands at 70 Whitehead Road.

Disclosures: Chair Kennedy: his son was a seasonal employee of the Golf Club
Member Creighton: has used a sign company owned by the Golf Club President
This is request to amend an existing SWP for the installation of paddle courts. They need to add
pavement around the courts to facilitate snow removal and allow for winter usage.

In attendance to represent agenda item: Jeff Hassett(Morse Engineering)
The property is owned by Phil Ayers who wants to remove fragmites. Pruning and landscaping has
already been performed near an intermittent stream along the property line. He needs to file an RDA
After-the-Fact for work already done.
8:15 PM: 61 Nichols Road—RDA#15-12 con't
The owner submitted a Notice of Intent for work to be done and is withdrawing-the.RDA without prejudice.

Recusal: Member Creighton cited personal reasons
The existing home is being demolished and rebuilt. There will be'a.minor increase'to the footprint and
additional impervious surface in the area of the drivevway. Abe tters'expressed concern over-runoff but
Paul Shea explained that a filtration system is being added to decrease.the amount of drainage and run-
off for abutters downgrade of the property.
MOTION: By Member Koines to close the.public hearing and issue a SWP for 20 Short Street.

The owner wants to build a.35'x 12' rear addition within'200""6f a riverfront and 160' of the buffer zone to
the riverfront. Paul Shea.said there..is an'approved Administrative Stormwater Review in place and that
the addition poses no impact to the riverfront.
MOTION: By'Member Marten to issue'a Negative 2 and Negative 3 RDA for work being done
within-a Resource Area at 191. Hull Street.

Carmen Hudson outlined how the owners want to add a second story to an existing structure with
cantilevered sections'of the-2na floor that will increase the footprint. They are also replacing an existing
patio with crushed 'stone and replace a 2°d story deck with a porch. The paved driveway will be replaced
by crushed stone which will reduce the current impervious surface by 100 slft. Paul Shea said all the work
is within the 50' buffer.zone and will require a variance but no SWP.A silt sock will be in place during
construction to protect the buffer zone.
MOTION: By Member Pimpare to close the public hearing and issue a Notice of Intent and
Order of Conditions for property at 401 Jerusalem Road.

8:20 PM: 33 Cedar Street—RDA#15-15
In.attendance to represent agenda item: Paul Caruso (landscaper); Mr. And Mrs. Millard (owners)
The Millards hired Mr. Caruso to do yardwork that increased the impervious surface but no SWP was
filed. Mr. Caruso said he assumed it was a non-issue when no one came out to the property but will
immediately file an After-the-Fact SWP.

According to Paul Shea, there is no consistency to the site plans being used by various departments.
Other inconsistencies include: a change to private water after-the-fact but no well permit filed with Board
of Health; lot line changes generated by the Assessor's office not being reflected on all sets of plans.
Richard Henderson said lot line Changes are not approved locally but through the Registry of Deeds so
that point is moot. PV feels the inconsistency with plans is an internal problem and not under the
jurisdiction of the Conservation Commission and that if proper permitting software is in place, much of this
confusion will resolve itself. She will pursue this with Town Manager Chris Senior.
