PLACE: COHASSET TOWN HALL—BASEMENT MEETING ROOM 41 HIGHLAND AVENUE, COHASSET, MA 02025 Board Members Present: Harvey, Dickstein, Vice Chair Lynn Doxey, Clerk Member Not Present: Robin Lawrence, Chairman Director of Public Health: Susan Sarni Public Health Nurse: Mary Goodwin Recording Secretary: Chrissie Dahlstrom 7:00 PM Dr. Dickstein opened the meeting. 7:01 PM—43 MILL LANE—EXPANSION OF USE—MORSE ENGINEERING The existing dwelling is four bedrooms. The owner would like to install a new Hoot System for a five bedroom dwelling. The peres have been completed. The Board didn't have any questions. MOTION BY MS. DOXEY: Motion to approve revised septic plans dated 3/10/2017 at 43 Mill Lane SECONDED: Dr. Dickstein VOTE: 2—0, Motion carries 7:10 PM—APPROVE MEETING MINUTES FROM MARCH 28 AND APRIL 10, 2017 MOTION BY MS. DOXEY: Motion to approve meeting minutes from March 28 and April 10, 2017 SECONDED: Dr. Dickstein VOTE: 2—0, Motion carries 7:15 PM—APPROVE THE WARRANT The Board reviewed and signed the warrant. 7:20 PM—PUBLIC HEALTH NURSE REPORT FINAL MARCH: Nursing visits: 69 Home visits: 5 Office visits: 64 Communicable Disease Reports: 1 Group A Strep Pneumonia Harborview: 7 clients Community Outreach/Meetings: • Weekly meeting to coordinate the Cohasset Health and Recreation Fair • Quarterly meeting of the Massachusetts Association of Public Health Nurses (MAPHN) Executive Board in Milton • Cohasset Care Program—one home visit • Monthly meeting of the MAPHN Southeast chapter in Hull to coordinate upcoming annual Public Health Nurse Conference on May 3-4, 2017 APRIL: Nursing visits: 55 Home visits: 4 Office visits: 51 Communicable Disease Reports: 1 Legionella Disease Harborview: S clients Community Outreach/Meetings: • Holly Hill Farm (HHF) site visit with the Cohasset BoH and staff from HHF viewed the mobile food truck and farm house kitchen • Attended the Harborview Housing Tenants Association monthly meeting and updated residents on the role and responsibilities of the public health nurse • Monthly meeting of the MAPHN Southeast chapter in Hull and discussed the opioid issue 7:25 PM—DIRECTOR OF PUBLIC HEALTH REPORT Holly Hill Farm: Ms. Sarni is working on getting the paperwork from HHF for the May 17, 2017 public hearing. She is waiting on: FINAL • Residential Kitchen permit for their Mobile Food Truck: Installing a dishwasher, refrigerator and crate for the dogs to be kept outside the kitchen • Temporary Food Permit for events:Application for sale of food during farm stand weekend events, still waiting on the paperwork • Farm to Table Events: Application for Farm to Table event on 5/20/2017 790 CJC HWY:The last video inspection revealed the first sag repair was fixed but uncovered a second sag. Due to the location of this sag a meeting will be scheduled with a pumping engineer to discuss installing a pump under the floor with a forced main through the existing building sewer to the manhole out front. 734 CJC HWY:A permit was issued to install the new system the beginning of April.The permit is good for two years. POOL APPLICATION—RED LION INN The representative for the Red Lion Inn was in a meeting with the Selectman and couldn't come to the BoH meeting.The Red Lion Inn has never had a pool permit.The Board would like to discuss depth markers,fence height and ensure the gates to the pool area are locked at all times. MOTION BY MS. DOXEY: Continue the Pool Application discussion with Red Lion Inn to the June meeting SECONDED: Dr. Dickstein VOTE: 2—0 Motion Carries NEW CONSTRUCTION—19 HERITAGE LANE—MORSE ENGINEERING In attendance for this hearing: Greg Morse, Morse Engineering; Christopher and Marilyn Fleming of 15 Heritage Lane Four test pits were done in the back yard by Neil Murphy in 2004.The proposed septic tank is gravity fed and is for a new four bedroom house. Both test pits comply with Title 5.They are not asking for a variance. Ms. Sarni doesn't see any issues with this new construction. The Planning Board has issues that won't affect septic design. Conservation Commission is reviewing the building plan for a Storm Water Permit. Mr. and Mrs. Fleming are concerned about the septic system being too close to the lot line.The septic plan complies with the lot line requirements.They were also concerned about the grade and Mr. Morse explained the elevation is being increased from 46 to 51. MOTION BY MS. DOXEY: Approve Morse Engineering septic system plan revised May 3, 2017 for 19 Heritage Lane SECONDED: Dr. Dickstein FINAL VOTE: 2—0 Motion Carries NEW CONSTRUCTION—21 HERITAGE LANE—MORSE ENGINEERING In attendance for this hearing: Greg Morse, Morse Engineering The proposed septic for the new home at 21 Heritage Lane will be in the back yard for a four bedroom gravity fed septic system .The test pits have been perced. In 2010 Tara Tradd approved the plan.There are no wells within 100'.The applicant is required to have 4' pervious soil, which is accomplished. The Board has no issues with this plan. MOTION BY MS. DOXEY: Approve Morse Engineering septic system plan revised May 3, 2017 for 21 Heritage Lane SECONDED: Dr. Dickstein VOTE: 2—0 Motion Carries 8:00 PM MOTION BY DR.
