SEPTIC REVIEW FOR 144 FOREST AVENUE (LOT 6A) In attendance for this Agenda Item: Dr. Pompeo; Jim Pavlik, Outback Engineering; Heidi Condon, HC Design All perc tests have been complete. MOTION: By Member Harvey Dickstein to accept the subsurface sewerage plan for 144 Forest Avenue dated 6/6/2013, revised 5/4/2016 SECOND: Chairman Robin Lawrence VOTE: 2—0 MOTION CARRIES 8:10 A.M. POTABLE WELL REVIEW FOR 144 FOREST AVENUE (LOT 6A) In attendance for this Agenda Item: Dr. Pompeo; Jim Pavlik, Outback Engineering; Heidi Condon, HC Design The plan shows a 100' radius around the potable well and is within the setbacks of all septic systems. The well will be dug prior to excavating the land for foundation. He will receive an approval letter from Ms. Sarni prior to well installation. MOTION: By Member Harvey Dickstein to accept the potable well plan for 144 Forest Avenue dated 12/16/2015, revised 5/2/2016 SECOND: Chairman Robin Lawrence VOTE: 2—0 MOTION CARRIES 8:15 A.M. APPROVAL OF REVISED REGULATIONS RESTRICTING THE SALE OF TOBACCO PRODUCTS The board discussed the new fine structure, restriction of flavored tobacco, and raising the minimum age to 21 for tobacco purchase. A notice of the approval will be in the Mariner. MOTION: By Chairman Robin Lawrence to approve the revised regulations restricting the sale of tobacco products in effect June 1, 2016 SECOND: Member Harvey Dickstein VOTE: 2—0 MOTION CARRIES 8:20 A.M. APPROVAL OF WARRANT 8:24 A.M. SET THE NEXT MEETING DATE Next meeting date will be Tuesday, June 21, 2016 at 7 P.M. in the basement in the Town Hall. 8:25 A.M. PUBLIC HEALTH NURSE REPORT: Nursing Visits: 72 • Home Visits: 9 • Office Visits: 63 Communicable Disease Reports: 0 Keep Well Clinics: • Harborview: 8 Clients Community Outreach/Meetings: • 4/6/16 Meeting with Cohasset Police Department, Board of Health and Safe Harbors, Cohasset Coalition, to finalize plans for outreach program to residents after emergency services are rendered for a drug related issue. Letter and pamphlet ready for distribution. Letter will be sent from Public Health Nurse and follow up visit will be with Public Health Nurse and a police officer. • 4/11/16 Meeting with Fire Chief Robert Silvia to discuss bladder cancer screening for current, call and retired firefighters.Testing will be voluntary and take place at the fire department on 6/6 and 6/8. Letter to be sent to all firefighters explaining program. Costs will be covered by the fire department. • 4/14/16 Attended Massachusetts Association of Public Health Nurses annual conference in Brewster Ma. Conference topics included Armed Intruder, Tick update, Battling super bugs with drugs from the sea, Ebola, "It Was Never About the Stuff", a hoarding presentation, from emergency to recovery. • 4/27/16 Coastal Coalition meeting with the Public Health Nurses from the towns of Hingham, Hull, Cohasset. Networking session with an update from each town on activities and programs dealing with the opioid crisis. 8:28 A.M. DIRECTOR OF PUBLIC HEALTH REPORT: Blu Mussel & Dunkin Donuts @ 154/156 King Street: Under Title 5 rules a septic report does not have to be reported to the BOH. Morse Engineering made component repairs to the system. The D box is equally distributing the dry and saturated field. If there is an overflow the leaching field will flow into the parking lot. Ms. Sarni has protocols in affect to safeguard the public and environmental health should this system go into failure. The protocols consist of having an emergency pumping contract and bi-weekly inspection by Greg Morse. If the system backs up, all parties involved must notify the Board of Health (BOH). New perc tests and plans will be submitted to the BOH. Currently the system has a passing Title 5. A certified order was mailed last week. Ms. Sarni will stop in once a week for an inspection. Fiori's Gulf& Dunkin Donuts @734 CJC Highway: Title 5 regulations states that if a system is pumped more than four times a year, it is a failing system. 734 CJC Highway has been pumping weekly for 6 months. Ms. Sarni wrote an order on 4/28/2016 with the Title 5 and Food Code violations stating the current system was not designed for a food Board of Health Meeting DRAFT 3 of 3 May 13,2016 establishment, which contributed to the failure. The new plan by Jack O'Leary of Sitec Environmental will replace the system by 7/1/2016. A certified order was mailed out last week. Beechwood Street/Mill Lane: Currently a failed cesspool is in the middle of the intersection of Beechwood Street and Mill Lane. The concrete is damp and it appears to be an abandoned system. Ms. Sarni called the DPW. Zika: Ms. Sarni is frequently updated by the State on the Zika Virus. Currently there is no concern that Massachusetts will be impacted. Public Health Education: Public education on melanoma and Lyme disease was done via Facebook, DPW education coffee and Health Fair. BOH has been distributing sunblock and bug repellent at these events. Tick warning signes from the state will be posted in Cohasset's school ball fields. Emergency Prep: The final items have been ordered using the Public Health Emergency Preparedness Grant. Cohasset BOH was awarded $4800 and with that funding the BOH was able to purchase sunscreen, field clothing, cots, billows, blankets, bug spray and emergency trauma kit for the town hall. 8:45 A.M.
