7:12 P.M. DIRECTOR OF PUBLIC HEALTH REPORT
90 Doane Street:
An anonymous letter was sent to The Patriot Ledger and Massachusetts Environmental Protection
Agency (Mass DEP). Susan Sarni spoke with Mass DEP and updated them that currently there are non-
existent public health issues. A letter was written to the Cohasset Mariner on September 23, 2015 in
regards to the same issue and there have not been any publications to date.
Joint Meeting: Sewer, Conservation, Water and Health Meeting:
2.02.16 Joint meeting discussing the plan study from GHD in the area of South Main Street
Permitted Food Establishment:
On February 4, 2016 the Board of Health (BOH) received a complaint regarding a food establishment.
The establishment was closed for the afternoon to clean and sanitize and was reopened at 4:30 of the
same day. A fine for the violation was issued.
Lead Testing by Cohasset Water:
Cohasset Water Department provided test results below the action level for lead and they will be
conducting the test again this summer at 20 sample sites.

Ms. Kirkpatrick recused herself from the hearing and left the room.
Items for this Agenda Item: Outback Engineering Plans Labeled with Each Address Below
Three new lots are proposed as 134, 144 and 146 Forest Avenue.
The proposed 134, 144 and 146 Forest Avenue properties wells are a 100' from all septic systems.
134 Forest Avenue: Outback Engineering plan labeled Subservice Sewage Disposal System LOT-5A, dated
6/30/2013, Revised 2/03/2016.
The septic plan is proposed to be made with Presby Environmental Pipe.The well is proposed to be
outside the 100' radius from all septic systems. Outback Engineering dug six test pits.
The BOH requires two test pits in the primary and two test pits in the reserve. Although there were four
test pits in each corner of the field,tests were in the over-dig. The BOH doesn't approve variances for
new construction. The BOH is requesting Outback Engineering reconfigure the field to avoid a variance.
The revised plan must show two test pits in the primary and two test pits in the reserve.
144 Forest Avenue: Outback Engineering plan labeled Subservice Sewage Disposal System LOT-6A, dated
6/06/2013, Revised 2/03/2016.
The septic plan is proposed to be made with Presby Environmental Pipe.The well is proposed to be

6/20/2013, Revised 2/03/2016.
The BOH requires the test pits to be located in the field.The field does not have to be in the shape of a
rectangle. The proposed plan shows a fence on the reserve soil absorption system. Title 5 does not allow
permanent structures in the reserve area.
