Meeting called to order 6:35 P.M. 18 Hillside Drive Septic Upgrade, Grady Consulting In attendance for this hearing: Rob Carlezon, Grady Consulting Material used for this agenda item: • Application for Disposal System Construction Permit dated June 4, 2019 • Septic System Repair Plan , designed by Grady consulting dated June 4, 2019 • Soil evaluation report dated April 29, 2019 Mr. Carlezon reviewed the septic repair plan for 18 Hillside Drive for the Board. The property is a lot with an existing 4 bedroom dwelling. There are wetlands in the back yard. The existing system is in the front yard and has failed a Title 5 inspection. Grady Consulting is purposing to remove existing septic tank portion of the existing leaching field, and install a 1500 gal septic tank, 1500 gallon pump chamber, d-box and a leaching field with infiltrator equipped for standard chambers. Grady Consulting is asking for two local upgrades.System will need 1 foot reduction to groundwater, 5 feet to 4 feet separation; and use a sieve analysis instead of a perc test.The soil was very sandy. MOTION: Dr. Lawrence moved to approve the local upgrade 15.405(1)(h) reduction in separation between bottom of soil absorption system and the high groundwater elevation from the required 5 feet to 4 feet. SECONDED BY: Dr. Pollastri VOTE: 3-0 MOTION CARRIED MOTION: Dr. Lawrence moved to approve the local upgrade of 15.405(1)(i)use of a sieve analysis where a percolation test could not be performed. SECONDED BY: Dr. Pollastri VOTE: 3-0 MOTION CARRIED MOTION: Dr. Lawrence moved to approve the septic repair plan dated May 31,2019 for 18 Hillside Drive SECONDED BY: Dr. Pollastri VOTE: 3-0 MOTION CARRIED 6:45 P.M. 42 Aaron River Road Septic Upgrade,Grady Consulting APPROVED In attendance for this hearing: Rob Carlezon, Grady Consulting, Marc Waegemann, property owner Materials used for this agenda item: • Application for Disposal System Construction Permit dated 6/18/19 • Septic System Repair Plan designed by Grady Consulting dated 6/19/19 • Soil evaluation report dated 5/2/19 Rob Carlezon, Grady Consulting, reviewed the Septic Repair Plan for 42 Aaron River Road. The property is located near the Aaron River Reservoir and in a nitrogen sensitive zone.There is a catch basin out in the street that discharges to a surface water supply, the Reservoir. The existing system is located in the back yard with the septic tank over on the neighbor's property. The existing tank will be pumped and filled. The proposed installation will have a Singular wastewater treatment tank for the reduction of nitrogen. The system will be gravity fed down to the back yard to a d-box and a leaching field consisting of infiltrator chambers.The system will have a silk sock that wraps around for erosion control. The local upgrade Grady is requesting is a setback reduction from the proposed septic tank to the property line. The setback is supposed to be 10 feet, Grady is requesting 8 feet. Also, a setback reduction of the leaching field to a wetland from 100 feet to 60 feet.Jason Federico,Cohasset Field Engineer, has some questions. Mr. Federico understands the setback issues but,would rather see the system closer to the catch basin then the actual reservoir. Mr. Federico feels the setback to the reservoir is more important to protect the town's water supply than a setback to the catch basin. Dr. Lawrence would like to continue the hearing and have Mr. Carlezon come back with revised plans moving the septic further away from the reservoir. MOTION BY: Dr.Lawrence proposed to continue 42 Aaron River Road Septic Upgrade Plan until our next meeting and have Grady Consulting come back with a plan if feasible if not we will have to go with the original plans. Approve the Warrant Members signed the warrant for 5/21/19 to 6/25/19 7:00 P.M. Public Health Agent Report for June 2019 Food: • Food vendors were permitted for the Farmers Market, South Shore Arts Festival, and the Triathlon. Ms. Fahey completed an inspection and re-inspection at Anchor and Sail to address problems with the refrigeration and food handling. Septic: • The 355 Atlantic Avenue homeowner with a cesspool adjacent to Sandy beach confirmed to Ms. Fahey that the connection to the cesspool has been disconnected and re-routed to the public sewer line as requested.Jason Federico is following up to make sure that the remaining cesspool is decommissioned by being filled with sand. • Friday May 31"attended Title 5 inspection at 380 CJC. Phil Spath also attended on behalf of the Town. It was determined that separation between the bottom of the system and groundwater was only 1.2'. At least 2' of separation is needed for the Septic Preservation System to work. Members of the Donald Staszko team suspected that there was a water line leak near the intersection of Sohier Street/CJC HWY that could potentially artificially raise the groundwater table. Last week a hole was dug again;the town water department collected two samples from the hole to test for fluoride, an indicator chemical for treated town APPROVED water. Both results indicated that the water in the hole was groundwater. Ms. Fahey called Brenda Douglas (Cohasset Water Department) and she confirmed the findings. She also said that the water pipes are located at a depth of 4.5' and water would have been present at this depth rather at 8' where the samples were collected. • Wednesday June 5th Ms. Fahey attended the MassDEP enforcement meeting with Mike Whiteside (Strikeforce) and Brett Rowe (Title 5)with Donald Staszko and his team of Jeff De Lisi (attorney), Jack O'Leary(engineer) and Bob Silva (septic preservation). Mr. Staszko admitted to adding hydrogen peroxide to the system in March. He denied trying to cover up his actions by adding snow on top of the d-box. Mike Whiteside will present his finding to MassDEP attorney Shaun Walsh who will decide the punishment, if any, for Mr. Staszko's actions. Mr. Walsh will also decide whether or not a groundwater discharge permit is needed for the 380-400 Plaza. Beaches and Pools: • Ms. Fahey conducted the annual inspection at the Town pool which opened on Father's Day weekend. Both the main pool and the wading pool were acceptable. Beach testing began. Rocky Beach exceeded the bacterial limit once, but samples were collected during an intense rainstorm with sheets of water cascading down Forest Avenue. A re-sample collected the next day was acceptable. • Shellfishing is prohibited in Cohasset and other shellfish beds from Boston to Plymouth until further notice per Massachusetts Division of Marine Fisheries. Red tide does not impact swimming. 7:05 P.M. Update on 380 Chief Justice Cushing Highway Ms. Fahey and Phil Spath, Engineer for the Town, went out to 380 CJC HWY to observe the observation holes that were dug to determine ground water elevations. Mr. Spath stated that observation holes were dug opposite one of the fields and hit ground water at 80 inches. Bottom of septic SAS trenches were 1.2 feet higher than the groundwater observed. Mr. Spath felt that if there was a leak in a water supply pipe, as Mr. Staszkos team thought, it would have come up from the pressure. Furthermore,the Town of Cohasset Water Department tested the groundwater at 380 CJC HWY for fluoride. Fluoride was not detected in the samples taken. It was not a pipe that burst causing the groundwater. Mr. Spath mentioned that in order to obtain approval for use of the White Night septic restoration system,you have to have at least 2 feet from bottom of the SAS stone to ground water, with a perc rate of 2 minutes per inch or greater; or 3 feet of groundwater separation with a perc rate of less than 2 minutes per inch.This system did not have the separation.There are I/A systems that could possibly be used but,the systems between Stop &Shop and Feng Shui building alone exceed 10,000 gallons.The systems would need a Groundwater Discharge Permit if there is common ownership of the properties involved. Bob Silva, Septic Preservation Services, representing Mr. Staszko had a question will the repair be under Title 5 or under a Groundwater Discharge Permit?This has to be determined before anything can be done. It is a legal question. Mr. Staszko is requesting Mr. Silva retest the depth of the water table. Mr. Silva feels the test results would be the same and the Board agreed. Moving forward Mr. Staszko's team will continue to send in monthly reports to Ms. Fahey and will have to wait to hear from Mr. Staszko's attorney and the DEP attorney to figure out what regulations Mr. Staszko will have to follow.
